How a Codex Run is counted — and every correction ever made to this Entry, published verbatim. Corrections are part of the product, not an errata footnote.
The counting rule
One authored thing is one item, decided by the game's own records. Every item has exactly one primary home — a chapter — and cross-references never double-count. Variants, permutations, and recolors are not items. Anything that cannot be bounded and verified is prose, not an item. Endless content completes at one of each distinct type. Branches count by experience, not by character. Achievements are first-class items. Any difficulty is allowed, but content that exists only on a particular difficulty must be captured where it exists.

The phantom-item kill rule
An item with no public evidence and no success on one thorough on-site check is retired to N/A with its reason published — never kept to pad a count, never silently deleted. Retired items stay listed as retired so the record stays auditable. This Entry carries 9 retired records.

Note: this log is tier-exempt by design. It cites item IDs and names the retired item, even at the Counts tier — no current item's name, hint, detail, or plate appears here.
8 October 2026 — Taxonomy rebuild (v1 → v2)
The checklist was rebuilt from the ground up against the game's own records. The rebuild re-derived every category count, merged duplicates, and re-verified each item's evidence. The Entry moved from the v1 taxonomy to v2: 507 records listed, 499 countable at the time, 498 captured, 1 mentioned, 8 retired.
8 October 2026 — Slide reel reconciliation
Frame-by-frame evidence split what the rebuild had merged and merged what it had split. The Abandoned Temple reel proved to be two distinct films — the incineration reel (sr12) and the vault-lowering reel (sr21) — with zero shared slides. The Damaged Laboratory's three experiment entries proved to be one continuous reel (sr05), confirmed by the ship log's own wording: “a slide reel that shows three experiments conducted with three different artifacts.” The River Lowlands theater reel was renamed to the vault sealing reel (sr02). Totals held.
12 October 2026 — nt128 session citation corrected (session 5 → session 4)
A Nomai text's session citation was corrected from session 5 to session 4 on its own evidence (the session-5 recording is too short for the cited timestamp; the harvested frame shows exactly the described text). On 24 October 2026 an audit found the correction had lived only in the evidence notes and not in the coverage file's structured session field, which still read session 5 — the field was corrected at the source, and the site data, spreadsheet, and Pack were regenerated to match. Pattern on record: provenance fields must be re-derived from the record whenever a correction publishes.
23 October 2026 — sr11 Secret Slide Reel retired as a phantom
The Secret Slide Reel (a “slide reel storage” room with an elevator-roof hidden floor in the Hidden Gorge) was retired to N/A at Scott's direction under the phantom-item kill rule. Exhaustive verification — all 31 session transcripts in both layouts, gorge-segment reads, a session-30 gorge frame pass, and the wiki's complete Hidden Gorge reel inventory, all already itemized and captured — found no such reel. Its sole source, a TheGamer listicle, apparently conflated the theater's stored reels, the burning-room elevator and discarded reels, and the laboratory's artifact film. The tome closed at 498 of 498.
